Complaint: In my XXXX and XXXX credit report, both had reports from First Federal Credit Control for {$85.00} owed. First Federal Credit Control also listed on the credit reports the same amount of {$85.00} with two different account numbers XXXX and XXXX. Double dipping on credit reporting. First Federal Credit Control never sent me notification of the debt for me to be able to review and dispute if necessary. I was just lucky to be viewing my credit reports for different reasons and happened to see the First Federal Credit Control reports.

Complaint: Office visit dated XX/XX/2017 billed XXXX $ was sent to the wrong health insurance. I called them to provide the correct insurance card and they got paid XX/XX/2017. XXXX Hospital Billing still reported me to the Collections. I called the Collection ( s ) Agency twice to correct the discrepancy but was asked to pay first. I refused as I have proof of payment. I called the XXXX Billing again and reported the discrepancy few weeks ago. I found out that they have not corrected the discrepancy, and because of this, I am getting higher interest rate for home refinancing.

Complaint: On XX/XX/2017 I had an outstanding balance of XXXX On my XXXX XXXX Credit Card. I paid off the full on the card and request that credit card be closed since I already had another XXXX XXXX Credit card. I was in the process of making a purchase about 3 months ago, when it came to my attention that the above mention Credit Card has a one 30 day late on XX/XX/XXXX and two 60 days on XX/XX/XXXX and XX/XX/XXXX. I called the Credit Card several time to try to understand why I was showing any late when I had paid off the account and closed it in XX/XX/2017. No one could not explain the reason why is this reflecting in credit. I called several times to try to get this error clear from my credit report, requested to speak to a supervisor but XXXX XXXX representative leaving me hold for a long time that I had to hang up the phone. In regards to First Federal Credit Collection. this is a collection that was paid by my employer because of an injury caused at work.

Complaint: M husband has been going to this practice for about two years. The staff and the office manager have reported t their collection agency teh wrong balance without providing them proof of the debt fully. On XX/XX/XXXX there was no calls nor correspondence in writing that we had an upcoming appointment with XXXX XXXX XXXX. our phone number had changed in XX/XX/XXXX. I tried to talk to the office manager several times and she was not willing and very agressive short of cussing me out. to correct the amount of the bill. I paid XXXX to the XXXX location office in XX/XX/XXXX. That is the only way that he could have been seen the last visit that I take care of the past due amount of XXXX. Mr. XXXX can not read nor write very well at all. I handle all of the financial matters. The office manager told the staff to hang up on me if I called. The charges of XXXX, XXXX, XXXX dollars are totally incorrect and according to the Fair Debt Collection Act are in dispute. Right now Mr. XXXX is very ill with onl XXXX XXXX XXXX XXXX. Again we had no written notice of an upcoming appointment. Within a ear anything can happen. They failed to put anything in writing to our postal address before this action.

The account is unknown to me so I contacted the company XXXX XXXX XXXX XXXX by letter delivered by certified mail on XX/XX/2018 ( see attached letter and certification of mail ) that was over 45 days ago and I have yet to receive a response. This company has failed to comply as per the Fair Debt Collections Practices Act and the Fair Credit Reporting Act 611 ( a ) 1 ( A ) Procedure in case of disputed accuracy. My request to XXXX XXXX XXXX XXXX regarding account number XXXX for a sum of {$20.00} is to provide via CFPB portal full validation or delete this unknown account from my 3 credit files. I have explained that this account is unknown to me and I require FULL validation pursuant to the Fair Debt Collection Practices Act and XXXX XXXX XXXX XXXX has failed to provide me under the CREDITOR /DEBT COLLECTOR DECLARATION the following : 1. Agreement with your client that grants you the authority to collect on this alleged debt 2. Agreement that bears the signature of the alleged debtor wherein he/she agreed to pay the creditor 3. Any insurance claims been bade by any creditor regarding this account 4. Name and address of alleged debtor 5. Alleged account number 6. Address on file for alleged debtor 7. Complaint: I have sent a debt validation notices to the creditor and only receive a verification of the debt instead of a validation of the debt. As per FCRA, a creditor is supposed to respond with the original creditors name, address, and account number and not report to the credit bureaus nor attempt to collect until they have responded. However, the creditor did not send the required information and is still reporting to the credit bureaus, which is in violation of the FCRA. My next step is to contact a consumer protection attorney unless the matter is resolved here.

Name still misspelled, at XXXX, I phoned XXXX. and spoke with " XXXX '' advising her that my name was misspelled and that may be the reason why they are having trouble with insurance. 4. XX/XX/2017 followed up conversation above with a hard copy letter and fax repeating error on their part and sent copy of XXXX Summary Notice. 5. XX/XX/2018, received a collection notice from " First Federal Credit Control , Inc. '' with threat that unpaid balance will be reported to Credit Bureau. 6. XX/XX/2018 filed complaint with CFPB.

Complaint: Upon checking my credit report I noticed a collection agency named First Fed Credit Control based out of Ohio reporting that I owe them {$340.00} for medical debt. I contacted this company and requested that they validate the debt by sending me a signed contract between myself and them showing that I owe this debt. After 30 days I filed a complaint with the XXXX XXXX XXXX advising of the above asking for a signed contract between first fed credit control and myself. They responded via the XXXX website stating that they do not have a contract with me. So Im trying to figure out why they are attempting to collect money from me that I do not owe them and have been reporting this to my credit reports. I want them to cease all collection attempts and remove from my credit reports.

Complaint: I filed numerous complaints for First Federal Credit Control in regards to a account of {$800.00}. The collection agency has not presented any documentation as evidence that I legally owe this alleged debt. First Federal Credit Control has provided a itemization of charges which is not what I requested and also a signed authorization form to release medical records which also not what I requested. I requested a signed contract or instrument bearing my signature proving that I legally owe the debt. The collection agency responded stating that there is no provision in the FDCPA to provide the signed document. However, under the FCRA, section 609 ( a ) ( 1 ) ( A ), the CRA are required by federal law to verify- through the physical verification of the ORIGINAL SIGNED CONSUMER CONTRACT-any and all accounts they post on a credit report. Also under the FCRA, unverified accounts must be removed and if the copy of verifiable proof is not able to be provided, they must remove the account. I have been requesting a contract bearing my signature because it is by federal law that one is verified by the CRA to prove that I legally owe the debt. If a signed contract can not be presented to me then one will need to be presented to the CRA by law. If a signed contract can not be provided to CRA then this account needs to be removed under the FCRA. The collection agency has already openly admitted to not having a signed contract to legally prove that I owe the debt but refuses to remove the account which violates the FCRA. Under the FDCPA it states that collectors are not allowed to give inaccurate information about the consumer. I filed numerous complaints about the CA providing inaccurate and incorrect information previously presented to me in a form of a invoice. However, the account was still not removed for its inaccuracies but instead changes were made to make me the only party involved which violates the FDCPA. The collection agency has been non compliant with my requests. They violated HIPAA by presenting documentation including my mother who has no relationship to this account. They violated the FDCPA by presenting documentation with inaccuracies and also violated the FCRA by not having a signed contract bearing my signature. As a consumer I still have rights to privacy and fair practices.Collectors must abide by the FDCPA and are not allowed to treat consumers in any way that is unfair.

Complaint: I filed complaints for First Federal Credit Control on XX/XX/XXXX,XX/XX/XXXX,XX/XX/XXXX in regards to an account of {$800.00}. Although the information that has been presented by the collection agency was inaccurate and incorrect and a violation of HIPAA instead of removing or deleting the debt for its inaccuracies and for violating my rights as a patient " changes '' were made in their records to make the documentation more creditable. The reporting of this debt was made prior to allowing me to validate it which may be a violation of the Fair Credit Bureaus Act, to which can allow me to seek damages from a collection agent. The collection agency REFUSES to provide what I am asking for. I have requested NUMEROUS times for a contract or an instrument bearing my signature as LEGAL proof of the alleged debt. I did not request a " itemization of charges '' to which the collection agency has sent to my home address. Once again, I am STILL NOT requesting a bill or a " itemization of charges '' of this account because that is not sufficient documentation. I demand for First Federal Credit Control to provide a contract bearing my signature as evidence of the validity of this alleged debt. I demand that the collection agency to be compliant with my request of providing the following documentation that I am requesting, a contract bearing my signature, as that is the only documentation that I will accept as a form of validation. Until I receive the documentation that I am requesting I will continue to file complaints.

Complaint: I filed a compliant against First Federal Credit Control on XX/XX/XXXX and XX/XX/XXXX advising the following : Providing a bill/ statement is NOT a form of VALIDATION it is a form of VERIFICATION which is NOT what I requested. Verification means establishing truth and accuracy. Validation means to provided the legal proof of this truth and accuracy. Proof of validation includes leases, promissory notes, contracts with an original signature. I requested full documentation of the validity of the alleged debt with a contract bearing my signature. Which the collection agency still has not provided. Although the validation notice was sent on XX/XX/XXXX to the right address, however, the notice arrived in my mothers name, which technically I did not receive and also which is a VIOLATION OF HIPAA. The bill provided by the collection agency on XX/XX/XXXX also listed my mothers name as a patient and under the insurance information which is a VIOLATION OF HIPAA. The information presented by collection agency is inaccurate. I am not on my mothers insurance and my mother doesn't have medical insurance currently. The documentation provided violates my rights as a patient. The collection agency stated that their records are correct and accurate which is FALSE. My mother should not be listed on this account at all for any reason. My mother should also not be included in the bill/statement provided. ALL DOCUMENTATION PROVIDED IS INVALID AND INACCURATE. It is the responsibility of the collection agency to make sure that the information provided by the original creditor was indeed accurate and correct which includes the insurance information to which they have failed.

Complaint: On XX/XX/XXXX I sent, via certified mail, a debt validation letter to First Federal Credit Control requesting to validate the alleged debt of {$800.00}. The collection agency received the debt validation letter on XX/XX/XXXX and responded with a bill/statement/ invoice which I received on XX/XX/XXXX. I filed a compliant through the CFPB on XX/XX/XXXX stating that providing a bill and/ or statement is not sufficient documentation of the alleged debt and that it was only a form verification and not validation as requested. In return, the collection stated that " The verification of the debt was done correctly as provided under the FDCPA. '' The bill that was provided by the collection agency on XX/XX/XXXX is inaccurate. The bill provided has my mothers name listed under the patient information section of the bill which is a violation of HIPAA. Also in the insurance information section on the bill my mother is also listed as the primary insured person which is incorrect. My mother has not had medical insurance for almost 2 years and I have not been under my mothers insurance for over 4 years. The collection agency DID NOT verify or validate the alleged debt correctly or accurately. All documentation provided is inaccurate data and violates HIPAA.
