Complaint: On XXXX XXXX, 2019 I visited XXXX in XXXX, Texas to establish an account. I was denied, per personal banker due to a hardstop from XXXX. I was informed that a fraud alert was placed on my record. I was a victim of identity theft and had notified the Federal Trade Commission and all credit reporting agencies. On the same day, I contacted XXXX and was informed that due to technical issues the company was closed. I tried several days and could not reach XXXX. Finally, I was able to speak to a agent at XXXX. I explained to the agent that I was a victim of identity fraud and wanted to lift the security freeze to establish an account at a credit union. I stated that I have all documentation for filing a police report, the complaint form from the Federal Trade Commision. I, also, mentioned that I had left my name, address and phone number on XXXX voicemail to receive the form to lift the security freeze. Per message, I would receive the form in 5-10 business days. As of today, XX/XX/XXXX, I have not receive the form from XXXX.

Problem with personal statement of dispute
Complaint: I disputed a report on XXXX a XXXX XXXX XXXX account it was deleted after 30 days under the FCRA and WV Law then a few weeks ago I pulled my XXXX report and that report was illegally reinserted back on my XXXX report this report is totally inaccurate and it should have never been reinserted after 30 days and its removed its supposed to never be put back on my XXXX report the bank had 30 days thats the law they didnt verify it it was removed and should have never been put back on file if this report isnt permanently removed immediately I plan to initiate a lawsuit against XXXX that report made me have a XXXX XXXX under dr care now causing me XXXX XXXX XXXX XXXX and XXXX I want it removed immediately or I am proceeding with a civil lawsuit in US District Court against XXXX also XXXX didnt tell me how it was supposedly verified they put in a letter a XXXX XXXX XXXX with a XXXX XXXX XXXX address as the verification I never been to Cali so how is someone in California verifying a XXXX XXXX I want this removed immediately!

Complaint: I called XXXX to inquire of a credit card someone attempted to open in my name and they wouldn't give any information other than asking and I foolishly gave them all my information. I am afraid of identity being stolen again. I am a victim of identity theft. The matter I assured them it was not me, for I have not written a since the late 1990 's. Please remove the items that pertain to me. I not ever received any correspondence of any kind such as phone or mail.
