Complaint: A company called Capio Partners is not providing me with validation that I owe them any money. They are reporting that I have an unpaid debt with them of {$370.00} but I have never done business with them and I have never received any letters or notices from them. The account number on my credit is : " XXXX ''. I only found out there was an account on my credit report when I checked my credit recently and it was pointed out that I had a collection. I 've done some research online and I have only come across negative reviews and complaints by other consumers who have been harassed and scammed by this company. I will not give my money to a company I do not recognize and who will not tell me why they want me to pay them.

Complaint: XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, Fl XXXX NOTICE OF PENDING LITIGATION SEEKING RELIEF & MONETARY DAMAGES UNDER FCRA SECTION 616 & SECTION 617 Please accept this final written OFFER OF SETTLEMENT BEFORE LITIGATION as my attempt to amicably resolve your continued willful violation of the Fair Credit Reporting Act regarding your refusal to delete all of the UNVERIFIED account information from my consumer file. <P/>Despite my three written requests, the unverified items listed below still remain on my credit report in violation of Federal Law. In the results of your re-investigations, you stated in writing that you verified that these items are being reported correctly? You still have not identified who in your company verified the accuracy of these accounts or how they verified them? <P/>As previously pointed out to you, XXXX Law Dictionary, XXXX Edition, XXXX, defines Verification as, Confirmation of correctness, truth or authenticity, by affidavit, oath, or deposition. Affidavit of truth of a matter stated and object of verification is to assure good faith in averments or statements of a party. <P/>What this means is that your company must be willing and able to produce a document that proves the account ( s ) in dispute belong to me and the information being reported is accurate and you must be able to do this in a court of law. Someone in your company must authenticate the documents in your files that were used to verify the disputed accounts and to do this they must have 1st hand knowledge of the alleged account ( s ) and all that has ever transpired for the alleged account ( s ), under oath and under the penalty of perjury. <P/>Your failure to provide me with verifiable proof required for your company to post the accounts listed below is evidence that it does not exist and therefore is proof that you cant properly Verify the accuracy of any of the disputed accounts. Please be advised that under Section 611 ( 5 ) ( A ) of the FCRA you are required to promptly DELETE all information which can not be verified. <P/>Due to your continued willful disregard of the law I intend to pursue litigation in accordance with the FCRA to seek relief and recover all monetary damages that I may be entitled to under Section 616 and Section 617 if the UNVERIFIED items listed below are not deleted within10 days. <P/>A copy of this letter as well as copies of the three previously written letters sent to you by certified mail with proof of delivery will also become part of a formal complaint to the Consumer Financial Protection Bureau. They will also be submitted as evidence in pending litigation provided you fail to comply with this offer of settlement. I am unaware of any outstanding medical bills that I possess, and I am seeking the name and hospital/medical provider to which I owe the debt and a detailed breakdown of the fees that I owe. <P/>Additionally, I am allowed under the Health Insurance Portability and Accountability Act ( HIPAA ) to protect my privacy and medical records from third parties. I did not give permission to any of my current or prior medical providers to release any of my medical information to a third party. I am aware that the HIPAA does allows the release of limited information about me but anything more is to only be revealed with the patients authorization. Therefore, my request is twofoldvalidation of debt and HIPAA authorization. <P/>Please provide breakdown of fees including any collection costs and medical charges. Provide a copy of my signature with the provider of service to release my medical information to you. Cease any credit bureau reporting until the debt has been validated by me. <P/>Please send this information to my address listed above and accept this letter, sent certified mail, as my formal debt validation request, which I am allowed under the FDCPA. Please note that withholding the information you received from any medical provider in an attempt to be HIPAA compliant can be a violation of the FDCPA because you will be deceiving me after my written request. Complaint: Last month, we filed a complaint with your office because Capio Partners reported three medical bills on my XXXX credit report. Prior to reporting the accounts, they did not supply me with a notice of my right to dispute. Based upon that, they apologized and agreed to remove the accounts from my XXXX credit report. To their credit, they lived up to their word, and the accounts have been deleted from my XXXX credit report. As of today, Capio Partners are now reporting these same accounts to my XXXX XXXX credit report. Once again, I have not received a notice of my right to dispute, and I am demanding that these accounts be deleted from my XXXX XXXX report.

The credit bureaus deleted these and now I seem them back on with out the proof ever being sent to me. These charges listed are not mine and it is fraudulent for them to keep reporting without 100 % proof and accuracy, which has never been provided! I am in the process of buying a home and this has been resolved once by the deletion of these items, only for them to reinsert them, it is not fair for them to be able to keep doing that. I have NEVER been provided with the original paperwork with information showing this

Complaint: Received medical treatment on XXXX / XXXX / XXXX from XXXX XXXX XXXX XXXX in XXXX , OH. Never received any billing on this. I now have a notice from a collection agency dated XXXX / XXXX / XXXX . I called the medical center directly on XXXX / XXXX / XXXX . Informed them they never billed me. Customer service states multiple bills were sent and she only named some dates in XXXX . Informed me they were sent to my current address of XXXX XXXX XXXX , XXXX OH. The problem is this address has only existed for mailing since XXXX XXXX as it is a newly created address. So their customer service is using an address they just updated in their system but would not provide a reason why they did not bill prior to XXXX or actually use my real prior address I provided to them, XXXX XXXX XXXX , XXXX OH. I asked if this is now reported on my credit. The customer service person refused to answer this and told me I had to discuss with the collection agency. XXXX failed on billing and is the responsible party to correct the issue. I am dealing with XXXX and not the collection agency. I expect XXXX to be able to answer and resolve the issue because it is their failure on billing. If they or the collection agency reported this to my credit then XXXX needs to take the corrective action to remove this report on my credit and acknowledge they failed to bill properly. The bill is for {$690.00} and the collection agency ( Capia Partners, XXXX TX ) is offering a settlement of {$590.00}. I am ready to pay the full {$690.00} as I would have if I was actually billed. I need XXXX to be accountable for what they have done to my credit and answer my question regarding my credit record. Customer Service at XXXX should not be refusing to answer my question on an issue they are at fault.
