Complaint: Between XX/XX/2016 to XX/XX/2016, I was referred to XXXX XXXX XXXX XXXX XXXX 's XXXX XXXX by XXXX, the company that handles the VA 's Veterans Choice program. The appointment was made for me by XXXX and I followed directions and showed up for my scheduled appointments. Whether it was due to the construction of the office going on at the time or a simple mistake, my claim was sent to XXXX underneath my Social Security number as I did NOT have my patients choice ID with me at the time and was told that my Social Security number was fine. It would have been except that the facility, XXXX XXXX, billed XXXX ( healthcare provider for XXXX XXXX, XXXX and dependants, ) who sent a denial statement that the treatments were not going to be covered for " the veteran '' underneath my Social Security number. This is because I am NOT enrolled in XXXX to begin with. XXXX sent both XXXX XXXX and myself a denial notice around a month after the first visit. XXXX 's notice does not contain the information that my claim was denied because I was not enrolled in XXXX, it simply states that the coverage for the treatment has been denied. I went personally to the XXXX XXXX office and explained that it needed to be billed to XXXX. I was told by the receptionist ( I did not get a name ) that she understood and I assumed that it would be corrected. After I began receiving notices that I owed a debt that my " insurer '' would not pay, I finally went to the off premises billing office who then attempted to send the bill correctly. However, because the XXXX account had not been posted to, the authorization had been sent back to the VA as if I had never shown up for my appointment. Because they billed XXXX, Patients choice, ( aka XXXX, ) thought that I was a no-show. When they finally sent the bill to the correct entity, XXXX 's authorization had been closed, giving another denial to XXXX XXXX. I have been to the billing department at XXXX XXXX multiple times, I took them the original authorization code and then later I took them a physical copy of the authorization which they have. They refused to do more then send a bill in order to square away the problem, which with no explanation or supervision was of course denied. The computers that handle claims seem to have only a yes, or a no answer. Since then I have been back to XXXX XXXX where the billing department can not find a copy of the original denial ( that was from XXXX, ) and when I took them the authorization number from XXXX ( the correct insurance ) it updated in their system and no longer shows anything except refusals and who my insurance currently is marked in their system. The billing department of XXXX XXXX first sent my " debt '' to their in house debt department, an entity called " XXXX, '' who refused to work with me and contact XXXX in person. XXXX ( part of XXXX XXXX XXXX XXXX ) has since sold the debt to XXXX XXXX. XXXX XXXX is refusing to contact XXXX, instead asking me to pay out of pocket a reduced amount and seek reimbursement. XXXX XXXX XXXX Medical Center either refuses to acknowledge their mistake or can not see the original billing in their system and has little interest in doing so, they have sold the debt and written it off. I can not find a way to have an investigator from XXXX XXXX XXXX Medical Center, Capio Partners or XXXX to get on the same page. If XXXX XXXX would contact XXXX and submit the bill again, there has been an amended authorization since XXXX opened up for billing. But if they do not get this taken care of in a timely manner then the authorization will again expire. I am being victimized by a Hospital that has sold a debt that was not payed due to their error and taken a right off, and I am being victimized by a debt collection agency that has no interest in correcting a mistake that is not theirs. This goes against everything that the Patients Choice program was created for and it is now on my credit report. I had plans to get married, buy some property and move to Tennessee after I finish college underneath the Veterans Affairs Vocational Rehabilitation program ; this debt that is not mine and was not my mistake has caused me to have to delay my future plans, potentially in a permanent manner and I hope that you can assist me with getting this corrected.

Complaint: In XXXX of XXXX, I XXXX XXXX XXXX XXXX XXXX in order to pay for any outstanding debt owed on my part as I was pregnant as of XXXX of XXXX and would n't be able to pay any debts thereafter. I would n't be able to pay anything once I gave birth because my employer was a small law firm that offered no XXXX Leave or XXXX XXXX XXXX, therefore, once it was time to give birth and recover I would essentially become unemployed. It is now XXXX/XXXX/XXXX and I am in fact unemployed due to complications with my delivery that required a lengthy recuperative period. To my surprise, I have received a bill from a collection agency for {$4100.00} that is apparently owed to XXXX XXXX. I have several problems with this recent turn of events, first, I repeatedly asked the representative at XXXX XXXX if the bills proposed to me were complete, and she confirmed that they were, I do not have over {$4000.00} to give to anyone. I should have known something like this would happen, during the transaction it was apparent that they neither cared about my XXXX or dilemma even though I was trying to do the right thing. Second, I do not take lightly my private and proprietary data being sold to third-party organizations that I have no contractual obligation to. Who are they to share the very same information that they are supposed to protect under XXXX law? Third, I made XXXX XXXX well aware of my financial hardship, and they have made no attempt to adhere to the XXXX XXXX Assistance Law ( HFAL ). I have been offered no assistance nor have I been given an opportunity to negotiate my outstanding debt. Their subsidiary is equally ruthless, the collection letters make no mention of my right to dispute the outstanding debt. I feel that I am being mistreated by this XXXX provider and collection company based on my age and XXXX and existing XXXX disorder, I often have to visit the XXXX facility because I have an illness that they apparently are n't skilled enough to remedy ... it appears that these companies are more concerned with cashing in on my XXXX than trying to help a member of its community that is truly in need.

This collection company states that this debt is from a hospital in Florida- XXXX XXXX XXXX , located in XXXX , Florida . I do not recall ever going to XXXX XXXX in XXXX and I did not live in Florida in XXXX of XXXX, which is reported on my credit report. Capio Partners has listed this debt as opened in XXXX of XXXX for Credit Limit/Original Amount : {$1700.00}. Credit bureaus have certified this information as true but I have never received any documentation that this is my debt. I have received no phone call, no letters- no documentation at all that this debt belongs to me. This violated the Fair Debt Collection Practices Act.

However, after reviewing my records i am unable to find any documentations of any contractual relationship between name of company and myself which makes you a person in title to enforce a commercial claim against me. This letter is not a request for verification or validation. This is a request for proof of contract to substantiate your claim. Provide me with a certified copy of an original contract with my signature specifically naming your company as a person entitled to enforce a commercial claim against me. Certification can be done through the presence of a notary public who doolie swears the copy made is in fact a copy of the original paper contract in question. Failure to respond and provide strict proof of contract will constitute your tactic agreement that you name of president and name of company are not entitled to force a claim against me. Complaint: I applied for a loan recently and got copies of my credit reports afterward. I found out that Capio Partners is reporting a medical debt to the credit bureaus twice, under two different account numbers. The first tradeline I see on my report is reported under Capio Partners as account number XXXX in the amount {$2600.00}. The identical debt is reported a second time under XXXX XXXX XXXX/Capio account number XXXX, amount {$2600.00}. As a result, my credit report shows {$5300.00} owed instead of the original {$2600.00}. Both are on my report simultaneously. I can not in any way see how it is legal to report the same debt twice under different account numbers. This is clearly impacting my score negatively and unfairly and a quick conversation I had with a consumer rights attorney leads me to believe it is not legal. On top of this, I should not have any kind of debt of this nature due to XXXX Hospital at all and certainly never received any bill or invoice from the original creditor indicating this.

Complaint: To whom it may concern : On XX/XX/XXXX I was notified through XXXX credit monitoring that a collection appeared on my XXXX credit report from Capio Partners , LLC for {$500.00}. After contacting the company, I submitted a credit to all three bureaus as this was not my debt nor a provider I was familiar with. After investigation, the items was removed from all three credit reports mid-summer. However, after the item was removed, it has reappeared under a slightly edited name on XX/XX/XXXX and again on XX/XX/XXXX. Instead of being listed as Capio Partners , LLC it is now appearing on my report as XXXX XXXX XXXX/CAPIO Partners. As reported previously, this is not my debt. The continued appearance of this on my credit report is causing undue harm as I am in the middle of a home equity loan approval process. The reporting not once but twice within 3 weeks caused an 88 point drop to my credit score. I believe the name change combined with reporting this incorrect item twice in less than a month is an abusive practice. This is not my debt and was previously resolved. I am hoping the Bureau can assist me in getting this resolved. I have reached out to all three bureaus for assistance, but I am concerned the company will continue this practice and just keep changing the name and submitting to my credit report. As this is not my debt, I want this error permenantly cleared. Thank you for your assistance.

Complaint: I recently pulled my credit report and noticed they were some debt reported by XXXX/CAPIO that I did not owe or ever heard of or ever had contact with. I contacted the company certified mail asking to validate this debt because its not mine. The company responded with a generic XXXX XXXX print out saying as verification. That is nit verification, if XXXX/ CAPIO does not have any documents with my signature agreeing to pay, they need to remove this debt ( XXXX ACCOUNT # XXXX ) of my credit report immediately. They violated FCRA by not providing me proper validation in regards to this debt.
