Complaint: XXXX XXXX XXXX has 11 accounts totaling of {$8000.00} on my credit report for my soon from XXXX my son had nj family care Medicaid at that time and I cant even check with Medicaid to confirm the dates because I have not had custody of my son they cant even speak to me. My son has been in his fathers custody. This information is on my credit report I attempted to solve this issue with XXXX and they told me to work it out with the father so he can help me pay the debt the father has a restraining order on me and I can not work anything out with him. I am disputing this and will get a lawyer if this is not removed. I will be filling a police report

Complaint: I'll provide as much information as possible to make my story clear. My name is XXXX XXXX. On XX/XX/XXXX, I was admitted to the ER and was treated and discharged the same day. I have insurance with XXXX, and I was later billed. I received a bill on or around XX/XX/XXXX from HCFS Healthcare Financial Services. The bill shows the following : ER Visit, Provider : XXXX XXXX XXXX, MD, debits {$500.00} on XX/XX/XXXX. The bill also shows " contractual adjustment '', a credit, of {$420.00}. This appears to be the medical insurance covering a portion of the bill ( XXXX is my medical provider ). The remaining amount due was listed as {$79.00}. I prompted paid the bill with my Health Savings Account ( HSA ) on or around XX/XX/XXXX, with a confirmation number of # XXXX. I assumed the debt was settled in full at this point. Approximately 2 years later, I am notified by my parents that they received a threatening letter stating that they ( HRRG, to be later named below ) would proceed with judgement ( lawsuit ) if I did not settle or dispute the debt within 30 days. This letter was dated XX/XX/XXXX, approximately 2 years after the debt was paid in full ( in my opinion ), without me being noticed of any subsequent action taken by either the insurance company, the hospital, the doctor, or any other party. I contacted the company Heathcare Revenue Recovery Group, LLC ( debt collector, also referenced above as HRRG ) on XX/XX/XXXX via my cell phone at approximately XXXX XXXX EST. I spoke to an individual that refused to accept my documents related to the bill I had paid and continued to threaten legal action. I requested a supervisor and discussed my documents and understanding of the bill calmly with her. She advised me to send in the documentation and they would review and return my phone call if any further action was needed. I explained that I would pay the bill, if they provided me a document showing why I owed the amount, and why it was different from the bill I had previously paid. We ended the call with my assumption of the debt being settled unless I heard back from them. I was not contacted again by the company until XX/XX/XXXX at approximately XXXX XXXX EST, where an individual began the same type of conversation about the debt and he explained that they ( HRRG ) have sent all the necessary documents, but would not reveal where they had been sent. I have only received the original bill ( mentioned in the first paragraph ) and the final demand letter, which threatened judgement ( mentioned in the second paragraph ). The individual explained that they would send me a letter ( showing the justification for the debt ) within 5-10 business days, which is something that I requested almost 30 days ago ( on XX/XX/XXXX ). I do not feel that I owe this debt, I feel that this debt is not valid, furthermore I feel that 30-40 days is an unreasonable due diligence timeline for a debt collection company ( HRRG ) that falls under regulation by the Consumer Financial Protection Bureau ( CFPB ). If the company has already sent the document showing the adjustment, it should only take minutes to reprint and send to me. To which I would expect to see the date of the alleged adjustment and justification for the difference in the discrepancy of the two bills. To advise a consumer to wait an additional 10 days, leaves me further worried that they will stall in sending the validation letter in order to proceed with judgement. I currently have no debt with any individual or company and a well above average credit score. I feel that these threats made towards me are unwarranted and unjustified. I will gladly settle any balance owed, if I can validate the money owed is in fact due to an insurance adjustment, that I will need to validate with the company, XXXX. Complaint: XXXX has a collection account on my XXXX credit report that was paid back on XX/XX/2016, I went to dispute this item to remove it from my credit report it came back not paid so I went to dispute this with them asking for them to prove that they own the debt to send me purchase agreement with a bill of sale or signed agreement with the original creditor then I ask for them to send me prove that I owe the debt like signed document they failed to comply to my request. I send them two certified letters and refuse to take it off my credit report. and they changed the account on credit report too.

Complaint: There is an issue with an old account that is reporting on my credit report that should not be there. I had an issue with an account that was related to an injury that happened in 2010, yet it is still reporting on my credit information. I have tried to investigate this with both the company ( XXXX ) and XXXX, yet there is an issue with this STILL showing on my credit report stating that I owe {$130.00}. This needs to be removed off my credit report, yet it is still showing up. All that I ever receive is these notices from XXXX stating that I owe this money that I do not owe. This is way past the statues of limitations regarding it showing up on my report. This alleged debt is over 9 years old, yet there is nothing that I have gotten from them that validates that this is correct debt. I had insurance at the time and this needs to be removed. I have sent SEVERAL requests to have this validated and still nothing has been resolved. This needs to be removed from my credit report at once.

Complaint: First of all I went to the hospital because of intense stomach pain, I had medicaid so it should have been covered.The original place was XXXX XXXX XXXX XXXX. The dates they had at XXXX XXXX XXXX did not match the date of the debt and I tried to sort everything out but it had already been a long time to find out why it didn't get covered as they said.I finally made an agreement with XXXX XXXX XXXX and they told me they would take it off my record if I paid an amount of XXXX on XXXX XXXX and not the XXXX as long as I made the payment that day.They lied and never took it off instead they sent it over to a different collection company I believe XXXX, still for the same amount of XXXX. I tried to dispute this but nothing was resolved.I have the receipt they sent me in my e-mail and I have proof in my bank account statement that they did get the money. Please look into this, I'm a single mom of XXXX kids and I was just trying to do the right thing and pay it this was a very big sacrifice for my family to pay that amount of money only to be robbed by them. Complaint: I was contacted by XXXX XXXX XXXX about a debt collection from charges allegedly originating on XX/XX/2018, when I was seen in an emergency room in XXXX XXXX. Their letter arrived one year after my visit and referenced an account with XXXX XXXX XXXX XXXX XXXX ( # XXXX ) for {$1100.00}. I never received even a single bill from this company. They are completely unreachable by phone, as I have made numerous attempts to acquire my original bill so I can pay the debt collectors if the money is indeed owed. XXXX provided me with no additional contact information. Over the past year, I have paid numerous providers in relation to this medical event, without issue. I have excellent credit and have never once missed a bill payment in my life. I can not believe it is legal for a company to contact me out of the blue with zero proof of a bill and demand a large sum of money. If they had simply provided me with ANY information, I would feel better about paying this money. However, their total lack of information leads me to believe I do not actually owe and this is some kind of scam.

They then put several accounts on my credit report. These actions are illegal according to the FCRA and I am demanding that all items be removed from my credit report as well as a check to be written to me in the amount of {$1000.00}, as is required by the FCRA for this illegal offense.
