Incorrect information on your report

Credit reporting, credit repair services, or other personal consumer reports: Credit reporting

Old information reappears or never goes away
Complaint: On XX/XX/2019 I sent certified letters to XXXX for two accounts listed on my credit profile. I mentioned that both the accounts werent mine. I also mentioned that the accounts seemed to be fraudulent erroneous and I sent in documents to present to show and the account was removed but following the next couple days the accounts were listed onto my credit profile. I believe that is completely violating my rights as a consumer and that could place this company or companies in jail for consequences of breaking and violating the law. I can show you how the company violated my rights. The account was placed on my account without my acknowledgment, I wasnt contacted by XXXX, XXXX nor any collections company. I stated and sent in certified verifying documents. You have now violated my rights through the FCRA the debt buyer or CRA have failed to comply with its obligations, I am able to sue for violations of the FCRA. I will be filing a lawsuit looking to get actual damages, punitive damages, and attorneys ' fees and costs of {$1000.00} per item. This is not a threat this is malpractice and violating my rights as a consumer through the FCRA and this matter will be taken up in court. I am only an intern dispute allowing less than 3 days for this matter to be completely and permanently removed and resolved. If you attempt to continue reposting these accounts I will be sure to contact law enforcement authorities, contact my attorney generals office, file more than one lawsuit. The two accounts that were listed and dates incorrectly stating that the account was opened in XX/XX/XXXX and in XXXX of XXXX and it was recently removed. I pay close attention to my consumer report and I receive alerts on my credit file and keep up to date to assure fools like you guys arent trying to dismantle my life placing erroneous debt on my credit file to make my life a living XXXX. I will be sure to place this online so that consumers are able to view this via online. I will also be filing a complaint as well. Law Offices Of Mitchell, was changed to MBA LAW. It was placed on my credit by the same collections company and that is a violations. Look at the dates it was removed and how quickly this account was placed back onto my credit profile. Well lets put it this way, this seems like a criminal act upon the credit bureaus and collections company. There was never a document sent to me to verify jack. I kindly ask for any documents bearing my signature a few months ago. Youre also violating my rights within the HIPPA laws as well.

Attempts to collect debt not owed

Debt collection: Medical debt

Debt is not yours
Complaint: This complaint is against the company MBA Law ( Mitchell D. Bluhm & Associates , LLC or MBA Law ). They claim that I owe on a medical bill in the amount of {$1600.00}, but I dispute that bill, because it is for services I do not believe I received, and there is no evidence that MBA Law is registered or licensed in the state of Florida to legally act as a collection agency. I left two voicemails, and sent two e-mail 's disputing the bill, and requesting proof that MBA Law had authority to collect the debt in the state of Florida. I received no response from MBA Law about this question and when I asked a representative on a later phone call to produce their FL license or registration, she simply said that it was public knowledge. There is no record of registration as is required by FL Statute XXXX, which requires the registration of consumer collection agencies. I called the FL Office of Financial Regulation, and they told me directly that MBA Law ( Mitchell D. Bluhm & Associates , LLC ) was NOT licensed or registered in Florida to try and collect this debt, and that they had multiple other complaints about them. The FL Office of Financial Regulation then directed me to file a complaint directly with CFPB. In addition, I dispute the medical bill because it is for services I believe I did not receive. After a car wreck that was not my fault ( uninsured motorist ), I had to have minor XXXX XXXX XXXX. I did extensive research and decided that I did not want XXXX XXXX, due to the much greater risk of side effects and increased recovery time. I only wanted local XXXX. I requested this of the surgeon and he agreed to do the XXXX where he would personally do a local injection to numb my knee. The procedure went very well, and never at any point did I receive additional services from an XXXX ( XXXX XXXX / XXXX ) due to pain or because I could not handle the situation. I was awake and watched the whole procedure on a video screen, while having a conversation with surgeon and his staff. They were amazed that this was my request, and said it was very rare to have someone choose XXXX XXXX rather than being put under. A month or more after the XXXX, I was under the impression that all bills for the medical services had been provided to me or my insurance company. I called The XXXX XXXX a couple times to confirm there were no more outstanding bills before I settled the case with XXXX XXXX and XXXX. We then settled the case, thinking all bills were paid. About four months later I received the first notice of a bill from XXXX XXXX , XXXX for {$1600.00} on XX/XX/2016. XXXX was the usual provider of XXXX XXXX for this arthroscopic type XXXX on behalf of XXXX XXXX XXXX. I think they did a routine billing without considering that I did not receive XXXX XXXX. I called XXXX XXXX multiple times to explain that the bill might be a mistake as I refused to have XXXX XXXX, and the surgeon from XXXX XXXX XXXX performed local anasthesia himself. As far as I know, I did not receive any services from XXXX XXXX. On XX/XX/2016 I spoke with a representative named XXXX XXXX XXXX XXXX and she assured me this bill was not going to collections, but being reconsidered. I called again one month later, and a different XXXX XXXX named XXXX told me on XX/XX/2016 that my responsibility was still {$0.00}, and the bill had been sent to XXXX, even though I disputed it. I also very clearly asked if it had been or would be sent to collections, and again I was told No. I made multiple attempts in good faith to rectify the situation. I asked for them to contact me directly if they were not satisfied after speaking with XXXX. I heard nothing else from XXXX. See my notes on the bill they sent me, and proof of XXXX XXXX Then three years later, I received a collection notice from MBA Law ( Mitchell D. Bluhm & Associates , LLC XXXX. Their paperwork shows that XXXX XXXX had already made a collection adjustment to their records on XX/XX/2016 and zeroed out my account. This contradicts the conversation I personally had with XXXX on XX/XX/2016 and on XX/XX/2016 and took notes on. It had not been sent to collections, and they were considering dropping the bill. They should have zeroed out the bill as an accounting adjustment, but should not have labeled it as a " collection adjustment ''. I believe that XXXX made some mistakes in billing me to start with and was disorganized in re-coding my situation, which resulted in this illegitimate bill being bought by MBA Law ( Mitchell D. Bluhm & Associates , LLC ), who bought the debt hoping to make money off of me, even though they are not licensed or registered in the state of Florida to collect, and I have never had any dealings or contracts with MBA Law.

Complaint: On XX/XX/2019, I exercised my rights under FCRA 611(a)(6)(B(iii) which provides that a consumer may request “a description of the procedure used to determine the accuracy and completeness of the information shall be provided to the consumer by the agency, including the business name and address of any furnisher of information contacted in connection with such information and the telephone number of such furnisher, if reasonably available,” regarding inaccurate accounts on my credit reports reported by XXXX/Law Offices of Mitchell Bluhm by sending Method of Verification letters certified mail, return receipt requested to XXXX, XXXX, and to XXXX. To date, I have not received any compliance/information/proof or verification methods to my request within the 15 days allowed from receipt of my letters/requests in compliance with the FCRA. Improper use of your report

Credit reporting, credit repair services, or other personal consumer reports: Credit reporting

Reporting company used your report improperly
Complaint: I previously mentioned that my name was leaked in a previous breach and personal information but that doesn't give you the right to use my credit or attempt to use my credit without my permission. By using third party, and collection companies and or credit companies paying out for information is a violation. You have violated my rights by providing incorrect on my credit report. I don't owe any medical bills at this time and never have owed in the past. Also, you have tried to sneak that same account unto my credit a few days after it was removed. That is clearly breaking the law and this matter will be taken upon court. I have the documented dates on all certified signature copies. This isn't a threat i am well aware of my rights and know for sure i am willing to take it to court to insure I'm payed for the damages and problems you have caused to my credit. Please note, I am providing you with an additional opportunity to have this account removed with this week deleted from my credit reports if you have not already done so. I have no desire to cause you unnecessary difficulty, however, this entry of my private health care information, on my credit report, for an account that no longer has ANY permitted business purpose waiver since there is NO payment due, has caused injury to my credit reputation, and has left me no choice but to proceed with the following : Upon my receipt of the FCRA and FACTA mandated reply from ( CRA ), if the account has NOT been deleted in its entirety, I will take appropriate action to enforce my rights under the HIPAA, FCRA and FACTA rules, ARRA and Florida 's Consumer Protection and Medical Privacy statutes. account : 1. Attempts to collect debt not owed

Debt collection: Medical debt

Debt was result of identity theft
Complaint: On XX/XX/XXXXthe Law Offices Of Mitchell reported a medical debt to my credit report that does not belong to me. This debt had already been disputed once with another debt collector and had been removed from my credit report, so the Law Offices of Mitchell must have bought out this debt. I have not been to a medical facility in at least 10 years. I do not have any liability to anyone else, for any medical payments. This debt does Not Belong To Me. I have had to dispute under identify theft before, with the credit bureaus. In XX/XX/XXXXMy vehicle was broken into and my checks and ss card/ID was stolen along with all of my business equipment. At that time I had to change my bank account information and issue a new ID & SS Card. As stated before, this does not belong to me. I haven’t even received a bill to call and dispute, I have already taken it up with the credit reporting companies. This needs to be removed from my report, and not sold to other parties. I am in the process of buying a home, and this debt is hurting my loan which is already in the process of closing XX/XX/XXXX. These people need to figure it out, if they have a copy of the information provided they should share. I don’t believe it’s fair I have to keep disputing the same debt, and the original company- XXXX XXXX XXXX Of Missouri and the debt collectors keep selling it to a new party.

Complaint: Received a letter from XXXX XXXX stating I owed a debt that I dont owe. They are using strategically misrepresentations of the facts to imply they are a law firm not a debt collection agency. I told them I was the victim of identity thief. I stated the debt was not valid and requested they produce competent evidence that bares my signature to prove I owe them a debt. Future more until they are able to produce valid documentation they do not take any future actions. The phone representative told me she could send me an invoice. I requested a signed agreement with my signature. She told me they had no legal obligation to provide such a document. Since the voice recording at XXXX states its a law office and the phone representative reaffirmed it, I told her to transfer the call or have the lawyer in the office call me. She told me no lawyer is going to call or is in the office.
